This survey is for an entity that coordinates administration of a program, such as a managed care organization, insurance provider, or an entity interested in submitting a proposal to administer Virtual PACE.       1. What opportunities exist for better coordination between service providers and improved integration of services? What modifications to the Medicare and/or Medicaid programs are necessary to enable improved coordination and integration?       8. What are the most critical issues the state should keep in mind when developing a program to coordinate acute, primary, long-term care, and behavioral services that are offered through Medicare and Medicaid?       9. Do you have other suggestions that would improve the quality and cost effectiveness of health care services received by individuals who are enrolled in Medicare and Medicaid?
